Welcome to the
System Safety Development Center
(SSDC™)
and the
Principles of Accident Investigation

Does the SSDC have the best system for investigating
accidents? You be the judge.
The primary intent of the SSDC's
system on the Principles of Accident Investigation is to
aid individual investigators and investigation panels in training for,
conducting, and reporting on investigations of accidents, in order to reduce the
probability of recurrence. (The same methods can be used for investigation of
any system deficiency.)
The SSDC's
system on the Principles of Accident Investigation
has been developed and
improved for over three decades. It was the first. It is internationally
recognized. (Virtually all of the other accident investigation systems have
spun-off from techniques developed by the SSDC.) Many people have taken this
SSDC course and proclaimed it the best course they have ever had. The SSDC
recommends that you study this material and make up your own mind - or take the
course and decide for yourself.
The Principles of Accident Investigation
training covers the basic principles of accident investigation, including
interpersonal considerations, collection of evidence, analysis of information,
report writing, report presentation and follow-up, and post investigation
activities. The training is designed to instill a behavior based safety ideology
in the participants and to create an attitude of urgency in preventing like and
similar accidents.
The
Principles of Accident Investigation workshop participant will be able to trace all the system deficiencies that
allowed the accident to occur, and to identify judgments of need, which if
corrected, will fix the system so that the actions of the people at risk are
changed and the accident cannot recur.
The Principles of Accident Investigation
workshop participant will recognize that property damage incidents and near
misses can herald an atmosphere, and organizational mind-set, that may be a
precursor to serious accidents and fatalities. Trends and common causal factors
in several incidents or accidents can identify links in the accident chain that
already exist - leaving personal personnel performance as the last, and only,
accident preventative.
The Principles of Accident Investigation
workshop is set up and priced to have the least impact on your budget. The
workshop is brought to you. This reduces or eliminates travel costs. All
subjects related to accident investigation, including witness interviewing,
barrier analysis, precursor management, use of the MORT graphical checklist, and change analysis are included in the
single workshop and the single fee, which is the lowest in the industry. (The
SSDC has lower overheads, and the savings are passed through to you.)

accidents? You be the judge.
